Your Free Occupational Therapy Exam Practice Question
A client with left homonymous hemianopsia is reading using adaptive strategies including the use of a vertical line on the left, and numbered lines on the left and the right of the page in order to stay on the correct line. In order to increase the client’s independence with reading, how should the occupational therapy practitioner grade the task up?
- Remove all visual cues for reading
- Utilize only the left vertical line on the page
- Remove all anchoring on the left of the page
- Utilize numbers only on the right of the page
The Answer and Explanation
Did you get it right? The correct answer is: B
Initially, when teaching visual search strategies for a client with left homonymous hemianopsia, a vertical line or anchor is placed on the left side of the page and the beginning and ending of each line is numbered sequentially. Once a client demonstrates improved visual scanning when reading, the task can be graded up by reducing the number of visual cues and eliminating the numbers on each line. When the task is graded up, the vertical anchor on the left side of the page remains.
Incorrect Answer Explanations
Answer A: Removing all visual cues for reading is advancing the task too quickly. Clients are developing new habits learning to read from left to right again with a reduced visual field, and the visual cues required to complete the task must be reduced slowly. Removing all visual cues will result in difficulty reading and frustration with the task provided.
Answer C: Removing all anchoring on the left side of the page is not recommended. The client has a left homonymous hemianopsia and is required to scan into the left visual field which is significantly reduced. The client benefits from the anchor on the left side in order to promote the return eye sweep and ability to find the start of each line when reading.
Answer D: Utilizing the numbers on the right are helpful at first in order for clients to learn to see the end of the line using the right visual field and match the corresponding number in the left side of the page using the left visual field. Reducing visual cues on the left side of the page reduces the reminders for clients to complete the return eye sweep and find the start of the next line when reading.
Bottom Line
In order to grade a reading task up for a client who is currently using two visual cues for reading, the visual cues should be reduced by eliminating one of the visual cues.
